The Dealers reservation form

This form gathers requests for Dealers' tables at World Fantasy Convention 2021 in Montreal, Canada, from November 4th - 7th, 2021.

The Contact First Name and Contact Last Name is for the person that we shall contact for further information.

Address: We ask for the business address of the dealer.

Desired no. of additional Tables: Please indicate the number of additional tables after the first you would like. We can guarantee one table for each approved application; more tables will depend on availability. 

Additional memberships required: Dealers tables come with no memberships attached. Up to 2 can be requested at a special dealer's rate.

Membership names: Names of the person(s) who will be staffing your table.

Preferred payment method: We will invoice you, and if you have a preferred payment method not on our list, please let us know. We currently accept Credit cards (via Paypal), Paypal, Physical cheques, money orders, Stripe, and Interac e-transfer.

Already have memberships: Please let us know if you already have bought the memberships for any or all of the named staff.

Comment: Please use to add any additional information about your application. If you have issues with the form please address them in a separate email to dealer's liaison.

Special requests: This can include requesting power (please specify the number of plugs), a special location in the general room, etc. All requests will be dealt with on a first-come-first-served basis.

Payment method: We will invoice you upon your acceptance of the dealer table offer. This should be sometime in August or very early September 2021. Please let us know your preferred method of payment in the field above.
